ASSUMPTION OF RISK & WAIVER OF LIABILITY

I, the undersigned parent or legal guardian of the above-named child, hereby acknowledge and agree that participation in dance and all related activities hosted at The Industry Performing Arts Studio involves physical movement and the inherent risk of injury. I voluntarily assume full responsibility for any risks of loss, personal injury, accident, or damage to my child and our personal property that may arise during participation in activities at The Industry. I release and hold harmless The Industry Performing Arts Studio, its directors, staff, volunteers, contractors, and affiliates from any and all liability, claims, or demands for injuries, loss, or damages incurred in connection with this event, whether caused by negligence or otherwise.

HEALTH & SAFETY

I confirm that my child is in good health and able to safely participate in all dance activities. I agree to inform the directors of any allergies, medical conditions, or special needs before attending the premises.

In the case of an emergency, I authorise the staff to administer first aid or call emergency medical services as required.

SUPERVISION & BEHAVIOUR

I understand that while The Industry Performing Arts Studio will provide supervision, I remain responsible for my child’s behaviour. I agree that any misconduct may result in my child being removed from activities at the discretion of the staff.
